US Post Acute Care, a physician-led practice group focused on post-acute medical care, has created a new management and analytics tool suite in partnership with Cosán Group.
Cosán’s team of clinical administrators use evidence-based risk assessments, chart reviews and remote interviews to create a snapshot of a patient’s health in between clinical visits. Integrating with US PAC’s SmartPAC clinical metrics reporting program, Cosán presents an attending clinician an integrated care plan with risk scores and key patient data points prior to a patient visit. The goal is to reduce administrative burden and help shape proactive clinical interventions.

Cosán has previously focused its technology-driven preventative care services on home care for older adults. CEO Dan Hirschfeld said he was intrigued by the idea of working with a clinically focused medical group with skilled nursing facilities.
