Two major players in the Institutional Special Needs Plan market announced new collaborations recently, bringing opportunities to providers in additional areas.

Longevity Health stepped into a joint venture with Infinity Healthcare Consulting, a multistate operator of skilled nursing facilities. The agreement marks Longevity’s entry into three new states, plus an expansion in Illinois. Beginning in 2025, Longevity and Infinity will collaborate to care for I-SNP members living in Infinity nursing facilities in Indiana, Kentucky and Tennessee.

As part of its partnership with Infinity, Longevity will deploy advanced practice clinicians, including nurse practitioners and physician assistants, plus medical directors, pharmacy experts, medical assistants and other professionals. 

The teams will engage with Infinity facility leaders and clinical staff, as well as primary care physicians, specialists, geriatricians, behavioral health experts, social workers and family members to create and implement coordinated care plans.

Similarly, in August, American Health Plans, a division of American Health Partners, announced a joint venture with Pennsylvania provider organizations Lake Erie College of Osteopathic Medicine, HCF Management and CHR Consulting Services, along with Highmark Health, to establish American Health Advantage of Pennsylvania.

American Health Advantage of Pennsylvania will begin offering the unique product to nursing home residents in 12 Pennsylvania counties.

The collaboration allows the provider organizations, LECOM, HCF, and CHR, to deliver the elevated clinical program of the I-SNP to their long-term care communities serving more than 3,000 residents in
Pennsylvania. 

Members of American Health Advantage of Pennsylvania will benefit from a coordinated model of care supported by a team of nurse practitioners or physician assistants from TruHealth of Pennsylvania.

Patient members also will receive support from a dedicated American Health Plans member advocate, who coordinates care and helps maximize plan benefits for members.

With the addition of Pennsylvania, American Health Plans will operate in 13 states.
