Northwest Michigan Long Term Care Specialists will join iNDIGO Health Partners, which will be the health services company’s first service contract outside the hospital.

Hospital patients will have smoother transitions into nursing homes or assisted living with the partnership, according to David Friar, president of iNDIGO. The company’s staff works from a single electronic medical record.

MLTCS specialists can focus more on patient care and have expanded resources, he said.

“We are responsible for patients 30 to 90 days after they leave, and one of the biggest problems has always been the risk of misunderstandings or lost information, so the closer we align these transitions, the better,” Friar said.
