One of the top cloud vendors in the long-term and post-acute care space is partnering with a data-exchange provider, aiming to smooth the transition from hospital.

PointClickCare Technologies announced this week that it is partnering with Redox Inc., in a move the two companies say will “work to bridge the gap between the acute and post-acute markets.”

PCC today offers a collection of cloud-based software and services that it says are designed to help LTC providers manage the complex needs of senior care. It says it is one of the first cloud vendors to partner up with Redox, which allows providers to exchange data information from one centralized hub.

“The transition to long-term post-acute facilities is becoming increasingly important for health systems as they look to reduce costs and improve care outcomes,” Luke Bonney, CEO of Redox, said in a release. “As a leader in the LTPAC space, PointClickCare is the ideal partner to help redefine the transition from acute to post-acute care by ensuring the data is not only effectively exchanged, but also harnessed to deliver vast improvements including facility matching and regional population health efforts.”
