Lacuna Health has bought American Chronic Care LLC a provider of chronic care management services to physician groups, accountable care organizations and federally qualified health centers.
ACC’s existing clients will now be a part of Lacuna Health’s clinical engagement model, the company said.
The new services are “a natural extension of Lacuna Health’s existing registered nurse-led clinical engagement solutions for our growing customer base of accountable care organizations, hospitals, post-acute care providers and physician groups,” said Brian Holzer, CEO of Lacuna Health.
Lacua offers outsourced CCM, allowing physicians to bill the Centers for Medicare & Medicaid Services when interactions hit 20 minutes or 60 minutes over a calendar month. Lacuna receives a portion of the invoiced amount.
“CCM has been shown to benefit payers, providers and patients and improve outcomes for people with chronic illnesses,” Holzer said.
