A new solution from data-activation company Innovaccer aims to improve patient outcomes through better referral management.
The Referral Management Solution streamlines patient referrals for care organizations by reducing leakages, providing better connectivity and boosting scheduling and patient follow-up capabilities.
Users can view and schedule referrals at the point of care without leaving the EHR. Innovaccer’s referral solution provides scorecards for specialists and ranks them based on specialty group, payers, and geography, among other factors. Machine learning-based algorithms create automated worklists to help providers prioritize referrals and send key details to healthcare partners.
It also provides a two-way communication channel with patients to send appointment information and solicit feedback.
