Fit4D, which offers technology-enabled support for those with diabetes, announced a partnership with Humana Inc. on Tuesday.

The Fit4D personalized coaching will be used for high-risk Humana Medicare PPO members in North Florida. The company, based out of New York, pairs a certified diabetes educator as a personal coach, along with a technology platform.

The coaching allows providers to fill in gaps of care related to motivation, support and education for those with diabetes.
