Provider Partners Health Plan has announced new affiliations with The Alden Network and Aperion Care of Chicago.  

PPHP will provide its Institutional Medicare Advantage Special Needs Plan (I-SNP) to 12 buildings in The Alden Network. Nine buildings in the Aperion Care plan are part of the Chicago agreement.

PPHP has enrolled more than 1,000 members in long-term care facilities in Maryland, Pennsylvania and Illinois. The plan allows a local provider care team to check in daily with members, with a goal of reducing hospitalizations.

“Our partnership with PPHP provides our patients with innovative, supplemental health care services along with access to seasoned clinicians and will most importantly provide patients with improved health outcomes,” said Alden Network CEO Robert Molitor.  “From the first facility we opened in 1971 to the more than 40 sites we operate today, Alden residents have come to expect excellence from us and we’re excited to offer additional ways to deliver extraordinary care in a changing healthcare environment.”

Aperion Care provides skilled nursing and other services in Illinois, Indiana and Missouri.
