myCareGPS, billed as “powered by nurses,” has entered the market. The goal is for a patient to connect to his or her care team around the clock.

The clinical management app, offered by ACCESS Nursing Services,  is meant to help providers reduce costs and ensure consistency. Features include integration of population health data, guiding care coordination through hospital discharge plans, minimizing community-based risks due to poor assessment, and organizing chronic care management revenue streams.
