Cardinal Health’s naviHealth Inc. has announced plans to buy Curaspan Health Group. The deal is expected to close by the end of June.

Curaspan’s software is geared toward transitioning patients from the hospital into post-acute care.

“Curaspan has built a leading technology solution for transitioning patients throughout the continuum of care,” said Clay Richards, CEO of naviHealth. “As we bridge the gaps among people, technology and processes, naviHealth’s primary goal is to enable the highest quality of care for patients across all stages of their post-acute care journey.”

Curaspan is in more than 600 hospitals and has more than 8,000 post-discharge customers including skilled nursing facilities, home health agencies, and long-term acute care hospitals, the company said.

Combining naviHealth’s post-acute management software, RightCare, and Curaspan’s technology will give the company “leading footprint in healthcare’s post-acute space,” said  Michael B. Petras Jr., president, Cardinal Health Post-Acute Solutions.
