A Phoenix post-acute development will be the first project operated by Mainstreet Health for Mainstreet.
The facility will have 94 private beds and will offer short-stay rehabilitation and therapy, as well as assisted living.
The project reflects a total community investment of $21.9 million, the firm said, and the facility should open in February 2017.
