More than $8 million in federal funding aimed at keeping people away from nursing homes started arriving in seven states Tuesday. Most of the money will be used to build hospital-discharge planning processes that emphasize home- and community-based care programs, officials said.
Aging and Disability Resource Centers (ADRCs) are programs that give information and help to individuals such as discharged hospital patients needing social services. Alaska, Kansas and South Carolina will enhance their ADRCs, while Missouri and Oregon will develop them with grant money.
Idaho, meanwhile, will focus on creating an individual-focused discharge planning program, the likes of which Wisconsin and Alaska are already trying out.
Centers for Medicare & Medicaid Acting Administrator Kerry Weems announced the funding, which is part of the Real Choice Systems Change grant program.
